Fire safety (2): design appraisal and acceptance dossier checklist
The three dossiers a factory investor needs: the fire police dossier (new Form PC11), the self-appraisal file (Form PC13) and the acceptance dossier (Form PC18).

In this article
The second article in our fire safety series lists the dossiers a factory investor needs: the fire police design appraisal dossier, the investor's self-appraisal file and the fire safety acceptance dossier. The lists follow Decree 105/2025/ND-CP as amended by Decree 347/2026/ND-CP (in force 15 September 2026) and Resolution 66.18/2026/NQ-CP.

Set 1. Fire police design appraisal dossier
For the technical design or shop drawings of works in Appendix III. Under Article 9(4)(a) of Decree 105/2025/ND-CP, as amended by Article 14 of Decree 347/2026/ND-CP:
| No. | Document | Notes | Check |
|---|---|---|---|
| 1 | Request for fire safety design appraisal on Form PC11 | Use the new form issued with Decree 347/2026/ND-CP | ☐ |
| 2 | Legal land documents or the investment or construction policy document | For example the IRC, the policy approval decision, the industrial-zone land lease | ☐ |
| 3 | Technical design or shop drawings showing points e and g of Article 16(1) of the Fire Law | Fire safety power; alarm, extinguishing, emergency lighting, evacuation, smoke extraction and pressurisation systems | ☐ |
- The dossier may be electronic or paper and must be confirmed by the investor and the design consultant. Paper documents are originals or copies presented with originals (Article 9(4)(c)).
- Information already in national or sector databases must be retrieved by the authority, not requested on paper.
- A person filing on behalf of the investor needs a letter of introduction or power of attorney and ID (Article 9(5)(a)).
- Compared with the rules before 15 September 2026, the new list no longer includes the investor's own fire safety appraisal document in the police dossier; the investor must still prepare its self-appraisal on Form PC13 (Set 2).
Set 2. Investor's fire safety self-appraisal file
Applies where the construction authority does not appraise the feasibility study (Article 8(1), Decree 105/2025/ND-CP). The result of appraising the contents of Article 16(1) of the Fire Law is recorded on Form PC13. The internal file should include:
- Fire strategy report: distances, fire access roads and hardstands, escape, fire resistance, compartmentation, smoke control.
- Fire site plan, escape plans and compartment plans.
- Fire and explosion hazard classification of each production building and warehouse (the basis for the Appendix III thresholds and QCVN 06:2022/BXD requirements).
- Construction design verification report covering fire safety (Article 26(5), 2025 Construction Law).
- The self-appraisal result on Form PC13.
If the design changes during construction, the investor must prepare the adjusted design and self-appraise it before work continues (Article 8(1)).
Set 3. Fire safety acceptance dossier
Under Article 8(3) of Decree 105/2025/ND-CP, the acceptance dossier for fire safety power and fire safety equipment and systems comprises:
| No. | Document | Check |
|---|---|---|
| 1 | Fire police appraisal document with the stamped design; adjusted design approved by the investor (if any) | ☐ |
| 2 | Investor's fire safety acceptance document on Form PC18 | ☐ |
| 3 | Inspection certificates for fire safety equipment (where applicable) | ☐ |
| 4 | Partial and overall acceptance minutes for fire safety systems and fire safety power | ☐ |
| 5 | As-built drawings of fire safety systems and fire safety power | ☐ |
| 6 | Operation and maintenance manuals for fire safety equipment, systems and related systems | ☐ |
Acceptance records for points a–đ of Article 16(1) (distances, access, escape, compartmentation, smoke control) follow the construction law, together with the completion acceptance dossier (Article 8(2)(c); Article 24, Decree 207/2026/ND-CP).
Final step: declaration on the fire safety database
Since 15 September 2026 the investor organises the fire safety acceptance, is legally responsible for the result and must declare it on the national fire prevention, fire fighting, rescue and fire alarm database before putting the works into operation (Article 12(1)(đ), Decree 105/2025/ND-CP, as amended by Article 16 of Decree 347/2026/ND-CP).
What changed in acceptance: see Fire safety (3): fire safety acceptance from 1 July 2026.
Practical tips
- Fix the fire hazard classification from the concept design; it decides whether the works need an appraisal at all.
- Keep one consistent set of fire drawings for all three procedures (fire appraisal, construction permit, acceptance) to avoid mismatches.
- Require fire equipment suppliers to deliver inspection certificates with each shipment.
- Run integrated tests (alarm, pumps, smoke extraction, fire doors) before the overall acceptance and record them separately.
